NY North Medical Clinic ("we," "our," or "the Clinic") is committed to protecting the privacy and confidentiality of your personal health information. This Privacy Policy describes how we collect, use, disclose, and safeguard your information in accordance with thePersonal Health Information Protection Act (PHIPA) of Ontario and thePersonal Information Protection and Electronic Documents Act (PIPEDA) of Canada.

We may share your information with:
- Other healthcare providers involved in your care (specialists, labs, hospitals)
- OHIP and authorized insurance providers for billing purposes
- Regulatory bodies as required by law (e.g., CPSO, public health reporting)
We will not sell, trade, or rent your personal health information to third parties.
6. Security
We implement appropriate physical, administrative, and technical safeguards to protect your personal health information against unauthorized access, loss, or misuse. Our electronic medical records system (Accuro EMR) maintains industry-standard security protocols including encryption, access controls, and audit trails.
7. Retention
Medical records are retained for a minimum of 10 years from the date of last entry, or 10 years after the patient reaches the age of 18, whichever is longer, in accordance with the regulations of the College of Physicians and Surgeons of Ontario (CPSO).
8. Your Rights
Under PHIPA, you have the right to:
- Access your personal health information held by our clinic
- Request corrections to inaccurate or incomplete information
- Withdraw consent for certain uses of your information (with some exceptions)
- File a complaint with the Information and Privacy Commissioner of Ontario
9. Website Privacy
This website does not collect personal health information, and nothing you type into a form on this site is recorded by any outside company.
Cookie consent. We only use measurement and advertising cookies with your permission. When you first visit this site, a banner asks whether to allow them; nothing described below loads or runs until you click Accept, and if you click Decline, none of it loads. You can change your choice at any time using the "Cookie settings" link in the footer of every page.
If you accept, we use measurement tools that help us understand how people find and use the site, so we can improve it and see which advertisements bring patients to us: Growify and Google Ads (advertising measurement) and TruConversion (page-layout analytics). These tools set cookies on your device — Growify's is kept for one year — and record the pages you view, the website that referred you, and your browser, screen, and time-zone settings. Cloudflare's traffic-statistics measurement runs regardless of your cookie choice, because it operates at our hosting provider rather than through a cookie set in your browser, and it does not identify you individually. Some of the accepted tools store information on servers in the United States, where it may be subject to United States law. Tools we use to improve how our pages appear in search engines and AI assistants work only with our public page content and receive no information about you. Like Cloudflare's traffic statistics, they run regardless of your cookie choice.
We never send your name, phone number, or email address to these companies, whether or not you accept cookies. When you submit the "Become a New Patient" callback request form, what you type is sent over an encrypted connection and emailed only to our clinic through a secure email service — it is not stored on this website and is never shared with advertising or analytics tools; the only thing the accepted measurement tools record is that a request was made, never who made it. You can also block or delete these cookies at any time in your browser settings; doing so has no effect on your care or on any request you send us.
