How to Book Your First Appointment

Bring Your Documents
On the day of your visit, bring your Ontario Health Card (OHIP), photo ID, and a list of current medications.

What to Expect
Your initial appointment typically takes 30-45 minutes.
Registration & Intake
Please arrive 10-15 minutes early to complete registration paperwork, including your personal and medical history forms.
Health Assessment
Dr. Soltani will perform a comprehensive health assessment including vital signs, physical examination, and review of your medical history.
Discussion & Care Plan
Discuss your health concerns, goals, and any questions you have. Together, you will develop a personalized care plan.
Follow-Up Scheduling
Before you leave, we will schedule any necessary follow-up appointments, lab work, or specialist referrals.

Walk-in patients are welcome at NY North Medical Clinic during regular clinic hours. While appointments are preferred to minimize wait times, we understand that some health concerns cannot wait. Walk-in patients are seen on a first-come, first-served basis between scheduled appointments.

What to Bring
Ontario Health Card (OHIP) and a valid photo ID. No referral needed.
OHIP Coverage Information
Most services at NY North Medical Clinic are fully covered by OHIP (Ontario Health Insurance Plan). You will not be charged for regular office visits, annual physicals, or medically necessary services.
Services with additional fees: Insurance medical exams, travel medicine consultations, certain medical forms and letters, cosmetic procedures, and uninsured services are not covered by OHIP and may incur additional charges. We will always inform you of any fees before providing the service.
